2018-2019 MMS Spring Sports Information
Prior to participating in MMS Athletics, ALL student-athletes and their parents must:

Parents select their child's sport (only parents can select activities and sports)

Complete all the required forms (Parents need to fill out sections and students will need to log in and electronically sign their forms as well.)

Submit a doctor's physical (PPE). The physical signed by a doctor must be turned in to the MMS office at the MI ⅘ building prior to participation. The physical will be valid for 1 calendar year from the date of the doctor’s signature.
NO PHYSICAL/NO FINAL FORMS=NO PARTICIPATION!
